Wavy lines in vision: when an eye specialist advises an OCT scan โ€“ Dr Rachana Tiwari, Chinchwad, PCMC

Wavy lines in vision usually point to a problem in the macula, the centre of the retina, not to your glasses. Dr Rachana Tiwari, eye specialist in Chinchwad, PCMC, checks the retina and often advises an OCT scan. See a doctor within a few days for new wavy lines, and the same day if vision drops suddenly.

“Doctor, the door frame in my kitchen looks bent.” In fact, I hear some version of this sentence in my clinic every few weeks. Usually the person has waited a month or two, because the eye does not hurt and reading still works. Moreover, the family often thinks it is just tiredness or an old number.

I am Dr Rachana Tiwari, consultant ophthalmologist at Chetna Hospital. So in this guide I explain what wavy lines in vision usually mean, when I send a patient for an OCT scan, and when I do not.

Why this is in the news now: around World Sight Day on 8 October, eye doctors urged people not to wave off wavy vision and other changes in sight.

What do wavy lines in vision usually mean?

First, straight lines bend when the light-sensing layer at the back of the eye is not flat. The macula is the small central part of the retina that gives sharp detail. If fluid, a thin scar layer or a small hole changes its shape, then straight lines bend. Doctors call this metamorphopsia.

The NHS lists straight lines that look wavy or crooked as a symptom of age-related macular degeneration. Also, swelling of the macula in diabetes, fluid under the retina after stress, and a thin membrane on the retina can cause the same sign. As a result, glasses alone rarely fix wavy lines in vision.

A pattern I see in my OPD

A pattern I see often. A man in his late fifties from Akurdi or Nigdi notices that tiles in the bathroom look uneven, but only with his right eye. In addition, he has had diabetes for years, and his last eye check was before the pandemic. Because his left eye is fine, the brain fills the gap, so he notices the change late. [Dr Rachana: confirm how often you see this pattern and the usual age group in your OPD.]

Similarly, I see the young IT professional with sudden blur and a grey patch in one eye after a stressful month. In both cases, the history tells me where to look, and the scan tells me what I am looking at.

When do I advise an OCT scan for wavy lines in vision?

After checking your vision and pressure, I dilate the pupil and examine the retina. Then I usually advise an OCT scan if any of these apply:

  • New wavy or bent lines in one or both eyes.
  • A blurred, grey or dark spot in the centre of vision.
  • One eye sees straight lines differently from the other.
  • Diabetes, or age over 50, together with any of the above.

What happens during an OCT scan?

Optical coherence tomography uses light, not radiation. First, you rest your chin on the machine and look at a small light for a few seconds. The scan then shows the retina layer by layer, so fluid or a hole becomes visible. Also, there is no injection and nothing touches your eye. [Dr Rachana: confirm whether pupil dilation is needed for OCT at Chetna Hospital and how long the visit usually takes.]

The American Academy of Ophthalmology notes that vision is lost faster with wet macular degeneration than with the dry type. For that reason, early scanning matters when wavy lines in vision are new.

When is an OCT scan not the first step?

However, not every blurry day needs a scan. For example, blur that clears when you blink often comes from dry eyes. Likewise, tired eyes after long screen hours and a slow change in your glasses number usually need a routine eye check first. In these cases I examine the eye, and only then decide on tests.

Also, if lines look wavy in both eyes only when you are very tired, the cause may simply be the tear film. Still, I prefer to see the eye once rather than guess.

How can you check wavy lines in vision at home?

The American Academy of Ophthalmology suggests an Amsler grid test, one eye at a time. You can also use a straight door frame or a line of tiles:

  • Wear your reading glasses if you use them.
  • Cover one eye and look at the centre of the line.
  • Then switch eyes and compare.
  • If lines bend, or a patch is missing, book an eye check this week.

In fact, doing this once a week takes thirty seconds. However, a home check never replaces a retina exam, especially if you have diabetes.

When are wavy lines in vision an emergency?

Go to an eye doctor the same day if you notice any of these:

  • Vision drops suddenly in one eye.
  • A dark curtain or shadow moves across your sight.
  • Many new floaters or flashes of light appear.
  • Wavy lines come with eye pain or redness.

After all, these signs can mean bleeding or a retinal detachment, so do not wait for a routine slot. To learn more, read about retina problems and when to see a retina specialist and why regular retinal check-ups protect your vision.

Wavy lines in vision: questions patients ask

Are wavy lines in vision always serious?

Not always. However, new wavy lines usually come from the macula, so they need an eye check soon. Then the doctor decides if you need an OCT scan.

Can stress or screens cause wavy lines in vision?

Screens can cause tired, blurry or dry eyes. However, they do not usually bend straight lines. So if lines look bent, get your eyes checked.

Is an OCT scan painful?

No. You rest your chin, look at a small light for a few seconds and the machine maps the retina. There is no injection and nothing touches the eye.

Can new glasses fix wavy vision?

Usually not. Glasses correct blur from the eye’s focus. In contrast, bent lines often come from the retina, which glasses cannot change.
