Anal abscess treatment in Chinchwad, PCMC starts with an examination by a surgeon. Dr Dhananjay Patil, general surgeon at Chetna Hospital, checks whether a painful boil near the anus is a fresh abscess that needs draining or a fistula tunnel that keeps refilling. A boil that returns at the same spot is rarely just a boil, so get it examined rather than wait.
“It burst, the pain went, and I thought it was over.” Many people say this about a boil near the back passage. Then a few weeks later the swelling is back at the same place.
[Dr Dhananjay: confirm how often patients come to your OPD with a boil that has come back two or more times, and how long they usually wait before coming in.]
Why this matters this week: Diwali falls on 8 November this year (Dekho Panchang). With travel, long hours of sitting and family plans, many people push a painful swelling to “after the festival”. An abscess that is filling with pus does not wait for the calendar.

Abscess first, fistula later: how the cycle works
An anal abscess is a pocket of pus near the anus. It usually begins as a painful, hot, red swelling, and some people get a fever. When it bursts or is drained, the pain eases quickly and the skin seems to heal.
NHS guidance explains what can happen next: most anal fistulas develop after an anal abscess (NHS). A fistula is a small tunnel between the inside of the back passage and the skin near the anus. Because the tunnel stays open on the inside, infection can collect again, and the “boil” comes back at the same spot.
So the boil that keeps returning is often the outside opening of a tunnel. Treating only the surface, with creams or home remedies, does not close it.
Signs it may be more than a simple boil
| What you notice | What it can mean |
|---|---|
| Painful, hot swelling near the anus, sometimes with fever | Likely an abscess. See a doctor the same day; it usually needs draining. |
| A boil that burst, healed, then came back at the same spot | A common sign of a fistula tunnel left behind after an abscess. |
| Pus or blood-stained discharge that stains underwear | A fistula can keep leaking from a small opening in the skin. |
| Throbbing pain that is worse when you sit | NHS lists this among the symptoms of an anal fistula. |
| Itching or soreness of the skin around the anus | Discharge irritates the skin; it is not just a hygiene problem. |
Symptoms in this table follow the NHS description of anal fistula (NHS). A high temperature with a painful swelling suggests an abscess and needs a same-day check.
What happens at the check
- Questions. When the swelling started, how many times it has come back, and whether you have bowel problems such as long-standing diarrhoea.
- Examination. The surgeon looks at the skin around the anus and gently examines the back passage. NHS guidance says a proctoscope, a small tube with a light, may be used to look inside.
- Scans, if needed. An ultrasound, CT or MRI scan can show the path of the tunnel when it is not clear on examination (NHS).
- The plan. A fresh abscess usually needs draining. A fistula usually needs an operation, chosen by where the tunnel runs.
[Dr Dhananjay: confirm which scans are done at Chetna Hospital and which are referred outside, and which fistula procedures (for example fistulotomy, seton or laser) you perform at Chetna.]
Why surgery is usually needed for a fistula
NHS guidance says anal fistulas rarely heal if left untreated and usually need surgery (NHS). The aim is to clear the tunnel and let it heal from the inside while protecting the muscles that control the bowel. The right method depends on how deep the tunnel is and how much muscle it crosses, which is why the examination and, sometimes, an MRI come first.
We explain the operations in more detail in our guide to anal fistula treatment in Chinchwad.
A pattern surgeons often describe
A man in his thirties gets a painful lump beside the anus. It bursts on its own after a few days and he feels better, so he does nothing. Two months later it swells again at the same spot, and this time it leaks a little pus every few days. By the time he comes in, it has happened three times. The examination finds a small opening on the skin with a tunnel running inside. Had the first abscess been checked, the cycle could have been explained at the first visit.
[Dr Dhananjay: confirm this pattern matches what you see in the OPD, or replace it with a pattern you see more often.]

Until you see the doctor
- Keep the area clean and dry; wash gently with plain water after passing stool
- Do not squeeze or try to drain the swelling yourself
- Do not rely on creams for piles; they do not treat an abscess or a fistula
- Come the same day if you have fever, spreading redness or pain that keeps getting worse
Other problems in the same area, and elsewhere
Pain near the anus can also come from piles or a fissure, which Dr Dhananjay’s OPD treats as well. A soft bulge in the groin is more often a hernia; see our page on hernia treatment in Chinchwad. For a soft, painless lump under the skin elsewhere on the body, read our guide to lipoma removal in Chinchwad, and for flank pain, our article on when a kidney stone needs a procedure. Dr Dhananjay’s OPD covers general surgery at Chetna Hospital, including laparoscopic surgery in Chinchwad.

Frequently asked questions
Why does a boil near the anus keep coming back?
A boil near the anus that returns at the same spot is often an anal fistula. NHS guidance says most anal fistulas develop after an anal abscess: the abscess bursts or is drained, but a small tunnel stays between the inside of the back passage and the skin, and it can fill with infection again.
Is an anal abscess the same as piles?
No. Piles are swollen blood vessels in and around the anus. An anal abscess is a collection of pus, usually painful, hot and red, and sometimes with fever. The two need different treatment, so a doctor’s examination is the way to tell them apart.
Will an anal fistula heal on its own?
Usually not. NHS guidance says anal fistulas rarely heal if left untreated and most need surgery. The type of operation depends on where the tunnel runs and how close it is to the muscles that control the bowel.
I also have a swelling in the groin. Is that related?
Usually not. A soft bulge in the groin that goes back when you lie down is more often a hernia. Dr Dhananjay Patil also offers hernia treatment in Chinchwad, and both can be checked at the same visit.
